Have one personal, one professional, and one pastoral recommendation completed using this form. Please note, recommendations must be completed by persons who are not related to you.

Step 4: Church Affirmation

Have your pastor or church leadership complete the Church Affirmation form found here.

Step 5: Application Fee

Pay the $35 application fee online
